In Network
I am currently in network with the following healthcare plans:
Aetna
Cigna
Optum
Out of Network/PPOs
If you have a PPO insurance plan for which I am not in network you may receive reimbursement for therapy. Upon payment after each session, I can provide you with a bill which you can submit to your insurance for reimbursement.
Since coverage will vary based on your plan, I recommend calling your insurance provider and asking about your out of network coverage.
Fees
If you are not using your insurance, out-of-pocket fees are:
Intake appointment: $215
Individual therapy (approximately 50 minutes): $195
Payment
The following forms of payment are accepted:
Major credit cards
FSA/HSA account cards
Cancellation Policy
I require a minimum of 24 hours notice for rescheduling or cancelling an appointment. A cancellation fee of $195 will be charged for sessions missed without such notification.
*Please note most insurance carriers will not reimburse missed appointments and you will be solely responsible for paying for any missed or late cancelled appointments (not just an insurance copay).
Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.
You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.
